Ambulatory Detoxification and Maintenance

We offer ambulatory detoxification. This means you will detox in the comfort of your own home rather than in a facility. You will need support, and it won’t be easy, but we will try to ease withdrawal symptoms and give you the highest quality outpatient treatment available.  This is at the doctor’s discretion.  They will meet with you on intake and decide if you are an appropriate candidate for outpatient detox. If not, they will refer you to detox and/or recommend options.

The way we address withdrawal symptoms will depend on which substance or substances to which you are addicted.

Alcohol—Strong medications such as valium will be prescribed to detox you if your doctor recommends this treatment. Other medications may be prescribed to help with the withdrawal as well.

Following the detoxification, we will offer you anti-craving medications, such as Campral, Naltrexone, Vivitrol, and/or the option of Antabuse.

Opioids, Pain Pills, Fentanyl, Heroin—Typically, we will prescribe Suboxone to help with the detoxification.  If you choose to stay on Suboxone for maintenance, this can be discussed at your doctor visit. 

Long-term MAT offered is Suboxone, Subutex (if appropriate), Zubsolv, Naltrexone, and Vivitrol.

We will also work with you if you want to switch from methadone to Suboxone.

Methamphetamine—If you are in a drug-induced psychosis, we can sometimes manage this in the outpatient setting to get you stabilized; otherwise, you may need to be referred to a higher level of care such as a detox or hospital-based location. Once stable, we can work with you on the long-term plan for medication management.

Anti-cravings for methamphetamine are being studied now. Research published in the literature indicates that N-acetyl-cysteine and Naltrexone have positive results.

Cannabis—We work with you to comfortably detox you from the withdrawal symptoms, typically irritability and insomnia.

Crack Cocaine, Cocaine—Managed with medications to make detox more comfortable, usually in the outpatient setting.

Benzodiazepine, Ativan, Xanax, Valium, Klonipan —Tapers are offered.

Other substances—please inquire or meet with your doctor to discuss. We offer help for nicotine dependence, kratom, tramadol, and other designer drugs.

Medication Assisted Treatment (MAT)

Kai Shin Clinic prides itself on offering same day appointments for those in need for MAT services. We offer outpatient inductions on suboxone, and will help to detox you from opioids, prior to starting on suboxone. We offer comfort medications to help with the transition to suboxone, as you must be in moderate withdrawal, before initiation of suboxone.  Please call us to make an appointment.   Drug testing and pill counts, close follow up is part of our program.  There is an expectation that you are doing some form of outpatient, or groups initially.  Treatment works best with medication and outpatient/counseling.

Psychiatry

We are not a typical mental health office. We do treat all mental health, but we specialize treatment for those who struggle with a substance use disorder, in addition to seeking help with mental health. Here at Kai Shin Clinic, our major motivation is to help those struggling with addiction.  Therefore, to be a patient here, you must have a history or active substance use disorder. If you do not, we will refer you to many other great mental health clinics in town. We make this distinction, because there are nuances to treatment, when considering both addiction and mental health, specifically with diagnosis, treatment and medication choices, and the special circumstances that affect this patient population. Our providers understand this interplay and connection, and work to support you in your recovery journey to better physical, emotional, and mental health.

Long term Sobriety

Clinically, long term sobriety means that you:

  • Have been chemical free for more than 12 months (with a clean urine drug screen).
  • Have a high-quality life.
  • Deal with typical stressors of day-to-day life using healthy coping skills.
  • Are responsible.
  • Attend parental, home, or professional duties.
  • Have a drive and passion for whatever it is that excites you: school, promotions, etc.

Relapse

Relapse is part of the addiction model. It is nothing to be ashamed of, but rather is something from which you can learn. If you do relapse, we encourage you to come forward to the clinic or your provider. We will individualize your care. It may mean increased visits, outpatient care, more follow up and support. Here at Kai Shin Clinic, we plan for and recognize relapses are part of the addiction disease, and we work with you to limit and minimize them through swift intervention, and services.

Mental Health Management

Many individuals who develop substance use disorders are also diagnosed with mental health conditions.  According to the National Institute on Drug Abuse, national population surveys have found that about half of those who experience a substance use disorder will also experience a mental health condition. When individuals have dual diagnoses or co-occurring disorders, the interactions between the illnesses can worsen the course of both. The high rate of comorbidity between substance use disorders and other mental health conditions calls for a comprehensive approach that identifies, evaluates, and treats both. Therefore, each patient will have a complete mental health evaluation.

Full mental health evaluation

A full mental health evaluation is a mental health diagnostic assessment. The diagnostic assessment (DA) is a clinical interview and written report that documents the clinical and functional assessment of a person’s mental health. The diagnostic assessment includes the nature, severity, and impact of behavioral difficulties, functional impairment, subjective distress, strengths, and resources. 

The clinical interview will obtain information on current symptoms and functioning and background information on social, medical, mental health, substance use, family, cultural, and developmental history. We use this information to identify any possible conditions that may interfere with mental health and make treatment recommendations. A person may also be referred for additional psychological testing to help clarify mental health diagnoses if needed.

MMPI and ADHD Testing

Psychological testing may be used to help clarify diagnoses and to guide treatment. 

Comprehensive and quality treatment starts with accurate assessment and diagnosis.  Treatment outcomes are improved as psychological testing guides treatment recommendations. Psychological testing, such as the MMPI, aids in assessing mental health conditions and personality symptoms. Adult ADHD testing is also available to determine if ADHD is contributing to an individual’s condition. If you are diagnosed with ADHD this can be treated on site here at Kai Shin Clinic.
